New Orleans, LA – Ochsner Digital Medicine has announced a strategic partnership with AmeriHealth Caritas Louisiana to offer remote patient management (RPM) services to members of the health plan. This collaboration aims to help individuals with chronic conditions such as hypertension and Type 2 diabetes better manage their health, reduce preventable emergency department visits, and avoid unnecessary hospital stays.
AmeriHealth Caritas Louisiana, part of the AmeriHealth Caritas Family of Companies, serves members across the state through the Healthy Louisiana managed Medicaid health plan. The program targets members who have experienced potentially preventable admissions, readmissions, or emergency department visits within the past 18 months. Eligible members can be enrolled in the RPM program by their care managers or referred by their primary care providers.
Participants in the program receive digital devices, such as blood pressure cuffs and glucometers, that connect to their smartphones or tablets. These devices allow members to submit readings directly to their Digital Medicine care team, which monitors the data for any abnormal trends. If concerning readings are detected, the care team, which includes a licensed clinician and health coaches, promptly intervenes to help members stay on track with their health goals.
Ochsner Health, a leader in digital medicine, has demonstrated the effectiveness of its RPM program, with 80% of participants achieving condition control within six months. This success has positioned Ochsner as a pioneer in using technology to address chronic health conditions efficiently.
