Our ministry has always focused on responding to the needs of the most vulnerable in our community. Today, we continue this over 150-year-old tradition by partnering with key stakeholders, community partners, and residents to help inform our response to their needs.
In 2020, we engaged in a system-wide process to explore ways to enhance our FY23-25 Community Health Needs Assessment (CHNA). This involved system-wide listening sessions with our Community Benefits Advisory Council (CBAC), Mission Leaders, and community partners. This dialogue was instrumental in designing a process that responded to community needs, aligned with our CONNECT 2025 strategy and identity as a ministry. As a part of the CHNA process, CHRISTUS Health will conduct its first system-wide community survey that incorporates the community’s voice by viewing it through a health equity lens that captures race, age, ethnicity, language, gender identity, sexual orientation, and Social Determinants of Health (SDoH).
We are building on the success of our previous CHNA cycles by enhancing the process with survey questions that are essential to recognizing the realities taking place with our most vulnerable population internally and beyond the walls of our organization. This is an exciting development for us as it allows us to live out our vision that we serve as a leader, partner, and advocate for health solutions. This process also includes Metopio, a platform that gives us data and health indicators and will allow us to benchmark and tailor data to our system. The survey results will help us prioritize the identified needs in hopes of improving the health and quality of life of the people in the communities we serve.
We recognize that we have limited resources and cannot respond to all needs; however, we will share data with our community partners to ensure they can have the most up-to-date information to respond to the needs of their target population. Working together, we will help build a network of care that ensures that all will experience God’s healing presence and love through our care.

• Identify health problems that affect the people in your community.
• Understand the needs of your community.
• Work together to find a solution.
